Waukesha Chapter of Wisconsin Alumni Association

Applicants Must:

-Be a graduating high school senior from an accredited Waukesha County High School in 2013

-Attend University of Wisconsin-Madison beginning the fall semester of 2013

-Be able to provide information regarding their academic achievements, extracurricular involvement, community service, financial need and community activities  (selection will be based on these criteria)

-Submit (5) collated packets to include:

• completed and typed application form
• transcripts with seven semesters of your grades, copies accepted
• two letters of recommendation
• (also include two self-addressed stamped letter sized envelopes)

-Must be mailed to:  Mary Komlo, Scholarship Chair, 628 Acorn Glen, Delafield, WI  53018- EMAILed applications are not accepted.

$1500

Deadline:

Must be postmarked no later than March 11, 2013
